The 3 C’s to Consider in Transitions

  1. Collaboration: Caseworkers are crucial in promoting collaboration, which starts the moment a child comes into care.

  2. Communication: Caseworkers model transparent, clear information sharing, particularly with the child and between caregivers.

  3. Continuity: Caseworkers ensure children can rely on the adults around them. This includes making sure their belongings go with them and there is contact between caregivers.
