When a youth is preparing for a move, we have the opportunity to respect their relationships and help them feel seen. This form is one tool to help caregivers and professionals understand what matters most during this transition. It’s voluntary and should be completed only if the youth wants to. Storybook Resource: Helping kids talk about transitions

One way to support children in transitions is to create a storybook about their experience. It helps opens up conversation between children, parents, caregivers, and workers, normalizes mixed emotions like worry, sadness, hope, and curiosity, and supports connection by giving adults simple language and a place for questions. Story creation can be simply drawing together or making something through apps or websites.
